Health and social care integration: making it happen

Our communities, the way we live, the health and social care we all want and need are constantly changing, yet the way we provide our health services has largely stayed the same for the last 40 years.

We recognise that an integrated care model can address issues of health and care fragmentation and, in turn, improve patient and service user experience. We also know that integration - when working as it should - can help support the delivery of better performance against targets. Integration even has the potential to help achieve financial balance and sustainability.

There is no doubt that significant activity is currently taking place across health and social care and there is a willingness to become more joined up. The various activities appear, however, to be happening discretely and in isolation from other redesign and reconfiguration efforts. Given the pressures in the acute sector, it is understandable that much focus - and resource - is going into tackling these very immediate challenges. Yet if we truly believe that a more integrated health and care service with local service provision is going to achieve better outcomes for our patients then we will need a paradigm shift. We need to think bigger and aim for an integrated system, rather than integrated services.
